It's pretty simple to upgrade, choose the time limit you want (1st step), click add to cart (2nd step), go to your shoppingcart and check this is correct (3rd step), continue to checkout (4th step), choose type of payment (5th step), then fill in your CC number if that's what you choose as payment type and click continue. (6th step)

These are way too many steps to be done "accidently" five or six time in a row.

Refunds costs us more money that you actually sent over to begin with (seriously there are two banks taking their cut from each transaction including the transacton of your money back to you), so we need a damned good reason to send off a refund as it actually puts us in the red.

That said, most people who think they upgraded five times didn't actually complete all the steps and have four pending upgrades which I can simply cancel. Now exhale - we can fix it.
